Repurposing Hospitals as Community Centers for Health

CAMA, Inc. in conjunction with Central Connecticut Senior Health Services of Hartford Hospital explored a new care model focused on health and wellness. 

Largely preventable chronic diseases such as heart disease, stroke, cancer, diabetes, obesity, and arthritis are the leading causes of death and disability in the United States. 

Conventional care models have only recently come under scrutiny for treating illness and injury as discrete medical events, rather than adequately addressing the root causes of disease.

To begin this project, we first conducted a deep dive into wellness practices to better understand the wide range of services and programs provided by communities, health systems, and retailers. 

Next, we tailored this search specifically to services relevant to an older population, which was driven by the demographic landscape of Connecticut. 

CAMA facilitated a team visioning session with major stakeholders to envision a menu of services targeting this niche market. 

From the visioning exercises, four pillars of wellness emerged, which were used to start a conversation around how a new model of care might reshape the Bradley campus.
